Psychological Service
As part of providing a psychological service to you, Jordan will need to collect and record personal information from you that is relevant to your current situation. This information will be a necessary part of the psychological assessment and treatment that is conducted.
Purpose of Collecting and Holding Information
The information is gathered as part of the assessment and treatment, and is seen only by your treating psychologist. The information is retained in order to document what happens during sessions, and enables the psychologist to provide a relevant and informed psychological service.
Cancellation Policy
If you need to cancel or reschedule any of your appointments, you need to contact your psychologist within 24 hours. This is because a consulting room has been allocated and reserved for you and your psychologist has committed this time to you.
Please give your psychologist at least 24 hours notice, otherwise you will still be charged for the cost of the session. Please note, you are not entitled to any Medicare or private health insurance rebates on cancelled appointments.
Confidentiality
All personal information gathered by the psychologist during the provision of the psychological service will remain encrypted, confidential and secure, except where:
It is subpoenaed by a court; or
Failure to disclose the information would place you or another person at serious and imminent risk; or
Your prior approval has been obtained to
a) provide a written report to another professional or agency (e.g. GP, lawyer, etc.); or
b) discuss the material with another person (e.g. a parent, partner or employer).
Access to Client Information
At any stage, you as a client are entitled to access all information about you kept on file.
